The Performance UV Lease Option General Terms and Conditions:
$375.00 for initial non-refundable placement fee
$125.00 shipping and handling
$99.00/month – with required 24 month contract, first month paid with machine placement
After 2-year term is completed, machine unit to be returned to Performance UV, or new lease term established. Options for extended lease agreements include month to month contract or renewed lease with included service support and replacement lightbulbs.
